I understand that participation in sports activities involves a risk of injury, and I agree not to hold Ummah Sports Academy or Ignite Soccer School liable for any injuries that may occur during the program

In the event of an emergency, I give permission for the program coordinators to seek medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

I give permission for my child’s photos and videos to be taken during the program and used for promotional purposes by Ummah Sports Academy and Ignite Soccer School.
